The Parkinson's Disease is the second most common neurodegenerative disease after Alzheimer's disease, with many implications not only on health but also on the quality of life of those who are sick. Unsurprisingly, research for more effective treatments intensifying. What, then, the new data?

H Parkinson's disease (PD) is, after Alzheimer's disease, the second most common neurodegenerative disease. Individuals suffering from the LA show motor symptoms such as tremor (shaking), slowness, stiffness, while there are often difficulties in walking and falls. People with PD exhibit non-motor symptoms, such as fatigue, sleep disturbances and memory, trouble urinating and orthostatic hypotension. Treatment of the disease is currently symptomatic, i.e., the drugs do not stop the progression of the disease, but relieves the symptoms. Levodopa (in combination with carbidopa / benserazide) is the most effective drug treatment for the disease. However, the initiation of levodopa therapy is usually later in the disease course. The reason for this delay is that the prolonged use of levodopa is associated with complications such as fluctuations in mobility (on-off) and hyperkinesias.
An important study recently published in the «Lancet Neurology» sends these data and indicates that the start of levodopa therapy need not be delayed. Of course always depends on the patient's age and severity of symptoms, and treatment should always be individualized. Nevertheless, this study is important as it reduces the "phobia" of levodopa treatment initiation, which is the most powerful and effective drug for the treatment of disease. It seems, indeed, that the emergence of chronic complications from the use of levodopa is mainly related to how high the dose administered and not on the duration of use.
New formulations
Recent investigations have focused on creating levodopa preparations, which leads to fewer long term complications. Such a formulation has been investigated in clinical trials and is expected to be released in the coming years. The new formulation contains levodopa sustained release (IPX066) and contains especially pellets designed to dissolve at different rates in the stomach and intestine. Also in progress are clinical studies with levodopa in inhalable form, for immediate relief from intense hypokinetic phases. Furthermore, in 2014 they published the positive results of a large study, a new drug for the treatment of hyperkinesia which constitute another important problem of chronic levodopa administration.
The research challenge
These preparations are expected to improve the results of treatment for PD reducing the undesirable complications. However, the great challenge of research to LA, is the finding of a neuroprotective treatment, ie a therapy that can affect disease progression. This year published promising results for gene therapy, however, need further investigation. It is clear that such treatments will be the future in the treatment of Parkinson's Disease, but for now the improvement of existing drugs, and therefore the quality of life of people with the disease is important.
